ATHLETIC DEPARTMENT FORMS
Prior to participation in the Terra Nova Athletic Department, each athlete and their parent/guardian are required to complete, sign, date and submit several forms to the Head Coach of their team. Some forms are general in nature and are required by all teams ... some are sport specific ... some relate to areas that support the athletic program (transportation, sponsorship opportunities, booster club membership, coaching employment applications, etc). Completed forms are kept on file by the Athletic Director and remain intact for the duration of the entire school year. Athletic forms must be filled out and renewed annually.

GENERAL ATHLETIC FORMS
(REQUIRED OF ALL ATHLETES IN ALL SPORTS) | |

Basic Athletic Form:
This form is required of all athletes in all sports. It includes parent permission for participation and transportation; verifies date of physical examination; verifies medical coverage or in absence of medical coverage, the purchase of the appropriate Meyers and Stevens Student Health Care Plan; emergency contact information; TNHS athletic contract. |

This form is required of all athletes in all sports (except football ... steroid form for football is included in football forms supplement). |

Photo & Video Image Release Form:
This form is required of all athletes in all sports. By signing this form, an athlete's parent/guardian/caregiver verifies the acknowledgement that their child's photo and/or video image may be published on Terra Nova Athletic Department or professionally contracted media productions. The form also provides an opportunity to request that an athlete's image NOT be used for any Terra Nova produced applications. |

Concussion Information Form:
This form is required of all athletes in all sports. The Concussion Information Form is mandated by California State Law ... it must be read and signed by both the parent and athlete. It explains the symptoms and signs of a concussion as well as the prescribed Concussion Protocol. |

TNHS PHYSICAL EXAM INFORMATION | |

TNHS Physical Exam Flyer:
This flyer contains information about physical exams for athletes. Every athlete must pass a physical exam prior to athletic participation. The physical exam must be taken during the calendar year preceding each sport's first day of practice. Information is provided for an alternate way to obtain a physical exam for those athletes who are unable to obtain a physical from their family physician. |

This form is required of all football players in addition to the Basic Athletic Form listed above. Forms supplement includes three sections ... Team Rules Acknowledgement, Helmet Warning Acknowledgement and Steroid Form ... each section requiring the signature of parent/guardian and athlete. |

Driver Certification Form:
This form must be filled out by parents, coaches and other adults wishing to volunteer the use of their personal vehicle for the transportation of TNHS athletes, other than their own children, to and from athletic events. Completed form must be submitted to the Head Coach, who will in turn submit the form to the Athletic Director. Reminder: Each volunteer driver must carry a copy of each student's ATHLETIC EMERGENCY FORMS in their vehicle while transporting TNHS students. |

Licensed Driver Under Age of 18 Form:
This form allows parents to grant permission for their licensed driver child under the age of 18 to drive only themselves to and from TNHS athletic events. |
